Whether applying for a new LLC or renewing an existing LLC follow these steps.

1. Please download and complete this form. LLC Application Form - pdf
   Once completed please mail this form to the Board of Examiners for Land Surveyors
   (address listed below)

2. You can either Pay online using the button above or you can send a check in the amount of $25.00 made out to the Nebraska Board of Examiners for Land Surveyors.
3. Upon receipt of your LLC Application form and $25 payment, the Board will prepare two LLC certificates and a receipt for your $25 payment. These items will be mailed to your business address.
4. When you receive the two LLC certificates from the Board you will retain one for your records and mail one to the Secretary of State for filing with the appropiate filing fee.
