2. Once completed please return this form to the Board of Examiners for Land Surveyors

3. You can either Pay online using the link above or you can send a check in the amount of $25.00 made payable to the Nebraska Board of Examiners for Land Surveyors.
4. Upon receipt of your Certificate of Authorization Application form and $25 payment, the Board will prepare a Certificate of Authorization and a receipt for your $25 payment. These items will be mailed to your business address.
5. The Certificate of Authorization must be renewed every year.
